GIFT CARD POLICY 

NEW PATIENTS

  • To redeem a new patient special offer, the offer must be mentioned when scheduling an appointment.
  • For new patient gift cards, the gift card will be sent by email after the appointment is completed.
    • Please provide a valid email address on your new patient paperwork to ensure you receive our offers!
  • Only one referral source per patient will be accepted.
  • Special offers cannot be combined.
  • Gift cards will not be sent after the end of the special offer month that the appointment occurred.
    • If you did not receive your gift card, please email smile@costasmiles.com before the end of the month that your appointment occurred.
  • Gift cards and discounts cannot be applied to appointments retroactively.

REFERRAL PRIZES

  • Each patient referral counts as one entry into our monthly drawing.
    • Patient referrals can be family or friends!
  • All of the referring patients will be entered and one winner will be drawn randomly.
  • Winners are announced within the first week of the following month.
  • One winner from each office will be selected and notified by email.
    • Gift cards will be sent by email to the winners.
